Please tell us some of your ideas (500 words max) for a transformation project you would like to complete during the fellowship. Some considerations may be a topic that is relevant to your work and addresses a known need in your practice. Applicants are encouraged to discuss the project topic ideas with your practice prior to applying. Projects related to opioids, obesity, and behavioral health are encouraged, but not required. As much as possible, please include an introduction, goal, and vision: Provide a brief overview of a transformation topic, an explanation of why it is important, and some initial thoughts about the project methodology. What is your goal for this project? What is your vision for the future and how will this project move you closer to your vision?
