fellowship self-assessment results

Before each cohort begins programming, fellows complete a self-assessment that explores knowledge, attitudes, and competency within the fellowship’s six content areas. At the conclusion of the year’s curriculum, fellows again complete a post-fellowship self-assessment to measure any change as a result of participation. health transformation projects

Each year, fellows undertake projects in their clinical practices that aim to improve quality and delivery of care to their respective patient population. After an initial needs assessment where fellows collect data from both provider teams and patients, each develop a scaled project that begins momentum in their practice toward continuous quality improvement. COMMUNITY LEARNING FORUMS AND DISCUSSION GROUPS

Throughout the year and once per month, various experts and leaders from the community were invited to speak about the challenges and successes of delivering their goals and objectives to the populations they serve in their respective agencies and organizations. The monthly discussions focused on various concepts within the six curriculum topic areas of wellness and burnout, quality improvement, medical education, substance use disorder, mental health, and social determinants of health. The fellowship has created a brief highlight reel below to provide a snapshot of these discussions that proved to have a lasting impact on all participants from faculty to fellows and the guest community panelists.
